• Our Nurse Communication Liaisons may be able to identify issues that must be resolved prior to discharge and assist in their resolutions.
• We explain our Healthcare Services in detail to patients and family members, ensuring realistic expectations of what Family Healthcare Solutions can and cannot provide.
• Patients and/or family members become knowledgeable, educated, and updated through our Nurse Communication Liaisons 24 hours a day, 7 days a week, which means fewer phone calls to physicians, nurses, discharge planners, case managers and social workers.
• All necessary clinical information is obtained by our Nurse Communication Liaisons. With facility and patient permission we will review the clinical record, contact the physician, and meet with the patient or family members to obtain all other needed information.
• Liaisons are also available for care conferences and senior meetings as needed. Family Healthcare Solutions provides services 24 hours a day, 7 days a week. Our corporate office hours are Monday through Friday, 8:30 a.m. to 5:00 p.m. CST. If you call outside these hours please leave a detailed message. Our 24 hour on-call service will be automatically paged and will return your call promptly.
