The process for initiating a healthcare service referral with
Family Healthcare Solutions is easy. Just follow these steps:
1. Contact one of our Nurse Communication Liaisons to meet and
evaluate any patient at any facility or send Family Healthcare
Solutions the patient demographics (name, room number, and facility),
anticipated discharge date, services required and physician contact
information via phone, fax or email.
2. Family Healthcare Solutions will notify you once the information
is received.
3. Provide patient with Family Healthcare Solutions contact information
so patient can have any healthcare questions answered.
4. Upon request from a facility, Family Healthcare Solutions
can place a multi-colored sticker on (or in) the patient’s
chart. This sticker indicates that the patient is a recipient
of Family Healthcare Solutions services and provides our contact
information. This simple sticker enables the discharge planner
to quickly contact us with any changes in the needs of the patient,
location to be serviced, or discharge date.
Once our Nurse Communication Liaison completes a family referral,
a copy of our referral form is left with the discharge planner.
This form includes pertinent patient information such as: conversations
with the physician, teaching reinforcement, and services ordered.
This form also contains contact information for Family Healthcare
Solutions office and the name of the Nurse Communication Liaison
assigned to the patient.
